All Balboa Students - General Annual Forms
To ensure that student information remains current and accurate, families are required to complete annual forms for each school year. Families who are unable to complete forms online will be provided with a paper packet upon request.
- The following forms can be completed online through the One Enrollment Portal at https://enroll.lausd.net
• Student Emergency Information Form
• Student Housing Questionnaire (SHQ)
- Forms that require wet (ink) signatures will be given to your student on the first day of school. You can find those forms below as well. Please complete all required sections and return the packet to your child’s teacher.

- Library Permission
Additionally, Google Parental Consent is required for use of Google apps in the classroom. This is accessible via the LAUSD Parent Portal. A Parent Portal account is required, and students must be linked to your account.
